information typed directly onto the form.

All forms should be accurately completed and/or printed using blue or blank ink.

which forms to select or which academic year you need them for, you
should refer to your Financial Aid Status Notification Letter,
review the forms descriptions below or contact the Financial Aid Office for further instructions.

Form Descriptions
Board of Governors
(BOG) Fee Waiver: to apply for a BOG only. When you complete the FAFSA (Free Application
for Federal Student Aid) for attendance at Reedley College you will
automatically be considered for a BOG.
Dependent
Verification Worksheet:
verifies family size and annual income information for student and
parent(s).
Independent
Verification Worksheet:
verifies family size and annual income information student (and their spouse, if married).
Educational History
Form: all financial aid
students are required to complete and submit this form. Collects
information regarding high school graduation status, previous
colleges/universities attended and ACG information.
Student/Spouse
Income Certification:
certifies other resources an independent applicant received to meet
living expenses not previously reported on the FAFSA.
Parent Income
Certification:
certifies other resources received by an applicant's parent(s) to
meet living expenses not previously reported on the FAFSA.
Parent Information
Request Form: to correct incomplete parent information for an applicant who must
provide parent information on the FAFSA.
Ability to Benefit
(ATB) Test: required for non-high school graduates. The form is to be completed and
taken to the Assessment Center for testing.
Selective Service
Registration/Waiver: either submit your information for Selective Service
Registration or waive registration, if eligible.
Drug Conviction
Eligibility Worksheet: used to determine financial aid eligibility for a student
who has a previous drug conviction.
FAFSA Signature
Page:
add a missing signature to a processed FAFSA.
Special
Circumstance Request:
used to request an income adjustment due to a special circumstance
such as loss of employment/benefits, divorce, death, etc. (available
in July 2008, for the 2008-2009 academic school year.) |
